As far as I am aware, Syzop has a HelpOp Login module on his website. It's about €60 and from what I have heard, works rather well.

In the UnrealIRCd.conf I think it is possible under the log section to log use of /vhost
Since I am against logging of any kind, I omitted this section and therefore cannot check :(

This sounds like f_auth by fez. I have used this module and it is quite good. However it does wreck havoc for users if you have set::auto-join enabled. As the user has to type the code, /part the channel and /join the channel.
