+B is NOT an oper-only usermode, funnily enough. Nor are any others you pasted, except +h - which as Jason pointed out can be set by services, etc.

This is digging my memory a bit, but... Maxbans is actually the number of bans you can have set on a channel. i.e.: *!*@* *!*@*.aol.com *!*@*.bigpond.com.au 3 bans.
