Strange Throttle Issue

Yesterday we had an issue with a bot attack and throttle. IRC Defender was at one point reporting 830 connections per minute (it's set to warn at 10), all from the same IP address (82.143.196.218 ). We have throttling enabled at 10 per minute. Here is a ~20 second snippet showing the received snotices and the shear volume of reconnects. Finally one of our ircops gzlined the IP and all was good. Another ircop tried reconnecting repeatedly and was throttled. I guess my question is what went wrong? I can provide portions of our set block if needed.
