Hi. the module returns the location in the "whois" twice. i have
checked the unrealircd.conf file and no line repeats

You may have changed the text "connected from" to "Conecta Desde " when the module was already loaded, or have set it differently on multiple servers. It can be cleaned by unloading the module (remove loadmodule from config and rehash) and then loading it back.
I wrote this explanation assuming you're using U5. It does not fit to the module for U4 though you've posted on U4 forum.

Is it possible to add the city to this module? I have the file GeoLite2-City-Locations-en.csv, could it be added?

This was already discussed multiple times. It is theoretically technically possible, but it would give extremely inaccurate data. The databases are very imprecise, and can't be made better, as sometimes a single IP address is assigned to or shared between users scattered across the entire country. Also (if you consider the data to be accurate anyway) it would create privacy issues. I won't do it.

I understand, although if we talk about privacy, revealing the country of origin of each user is also private, that's why I think this module should be limited only to operators, or add the option of cities so that only operators see it, if not bad inspired has something like this
