This is what you would do on XP Pro - 2K3's way of doing it would probably be similar:
Go to Folder Options. In the View tab, make sure that Simple File Sharing is turned off.
You are now able to right-click on oper.db, select Properties, go to the Security tab, and make sure that the user that UnrealIRCd runs as has the necessary permissions to that file.

Well, the only thing that I know of irc related that would have a file called "oper.db" would be services like Anope which would contain OperServ settings such as SQLINES, AKILLS, etc.
